As a client of Medicare Mindset & The Med Plan Store, our agency is here to assist you with your stand-alone Part D plan review for the 2025 Annual Enrollment Period (AEP), which spans October 15th - December 7th.
Please follow the 3 steps detailed below to complete your Part D plan (PDP) review.
NOTE: you can only change your Part D plan between October 15th and December 7th (if Appropriate)

How to Create a My Medicare Account
Step #1 - Create your Medicare.gov Account
Creating a My Medicare account at Medicare.gov only takes a few minutes. Here’s what you’ll need to create the account:
Access to the internet with either a computer, laptop, tablet, or smartphone
Your Medicare # from your red, white, and blue Medicare card
Your Part A (hospital) start date from your Medicare card
Your email address
After you create the account, move on to Step #2 below.
How To Update Your Medications On Medicare.gov
Step #2 - Add your medications & pharmacies
It’s very easy to add your medications and pharmacies in your Medicare.gov account. This video shows you how.
Then you’ll be prepared for Step #3, which you can start working on in October 2024.
How to Choose Your Part D Plan on Medicare.gov
Step #3 - Review plan options & confirm your 2025 Part D plan
This video will help you confirm whether your existing plan is suitable for 2025, or if changing plans is beneficial.
IMPORTANT NOTES:
You can begin performing the review on Medicare.gov for the 2025 plan year as early as October 1st, 2024.
If appropriate, you can change your Part D (PDP) plan between October 15th and December 7th. The new plan would begin January 1st, 2025. And your old Part D plan will AUTOMATICALLY cancel at the end of the year.
If it’s determined that your current PDP plan is still appropriate for 2025, take no action. Your plan will automatically renew January 1st, 2025.
